E-File allows contributors, service providers, and beneficiaries of program support to electronically enter data, process invoices, submit, verify, and certify FCC forms. E-File also supports Company Officers and General Contacts in performing administrative tasks, such as resetting passwords and managing entitlements.

I Forgot My User ID or Password

If you are a Company Officer, General Contact, Preparer, or Rural Health Care Program applicant and have forgotten your password, you can reset your password online. If you are an Authorized User, you must contact the Company Officer or General Contact that created your account to reset your password.

Please Note:

  • Your user ID is your email address. If you forgot your User ID, call USAC Customer Operations at (888) 641-8722.
  • When resetting your password, enter the temporary password that USAC sent you in the "Old Password" field.
  • Sharing your user name and password is a violation of USAC’s terms and conditions.

Schools and Libraries Program

FCC Form 472
Find form information or deadlines on the FCC Form 472 Filing page. Applicants will need to file an FCC Form 498 in the E-rate Productivity Center (EPC) to provide banking information and obtain a 498 ID for direct reimbursement.
