The Wireline Competition Bureau of the Federal Communications Commission announces the release of the revised FCC Form 498 and associated instructions to be used by the Universal Service Administrative Company (USAC) to collect contact, remittance, and payment information from service providers that receive support from the Federal universal service support mechanisms. The Office of Management and Budget (OMB) has approved the revised form and accompanying changes.
On Monday, November 17th, the form (also known as the Service Provider Identification Number and Contact Information Form) will be made available on the USAC website. In addition, service providers now have the option of completing the form online. Upon completing the online submission, the service provider must print, sign and return the forms to USAC to be officially certified. USAC encourages service providers to take advantage of the online submission capabilities.
