Eligible health care providers may submit forms for Funding Year (FY) 2012 and FY2013 using My Portal.
For the Healthcare Connect Fund (HCF) Program, currently only the FCC Forms 460 and 461 are available for online submission through My Portal. The other HCF Program forms will be made available for online submission on a rolling basis. Instructions for filing all HCF Program forms can be found on the HCF Program Forms page.
To access information from Funding Year 2011 and earlier, applicants may continue to log in to the Legacy system here.
Starting in FY2012, service providers now have the ability to submit and manage all invoices using the new service provider section in My Portal. For assistance getting access, contact USAC Customer Operations at (888) 641-8722 with your Service Provider Identification Number (SPIN).
