Look up funding commitments (FCC Form 462 and 466) information for both the HCF and Telecom Programs.

Look up request for services (FCC Form 461 and 465)  information for both the HCF and Telecom Program. Please keep in mind that this information is updated daily, so if your requests for services was approved today, it will appear in the Open Dataset tomorrow.

The Request for Services data set is also available in the USAC Open Data Portal. In this data set you can view both HCF and Telecom program information for FCC Forms 461 and 465 for current and past funding years. Please note that the data set contains the previous day’s data.

Look up your RHC Program deadline, which is six months after the end of your funding commitment.

This tool helps health care providers determine the status of their FCC Form 466. Use the tool to learn if USAC has received the form and collected the information it needs to make a funding commitment decision. If we are missing information, you will receive an Information Request once your form has been assigned to a reviewer.

Field labels and archived status reports

Enter your site’s address to determine the rurality tier and if you are in a Medically Underserved Area/Population. In the case where demand exceeds available funding, USAC will prioritize funding based on the following rurality tiers and those areas in a Medically Underserved Area/Population (MUA/P).

This tool allows service providers to view service request information posted by consortia and individual health care providers applying for funding through the Healthcare Connect Fund Program.
