FCC Form 484 Part 1
The FCC Form 484 Part 1 will be open from September 17 to November 1, 2024. The form asks applicants questions about the equipment and services they intend to fund if selected for the Pilot Program. These selections are informational, and applicants will not be bound by these selections if selected for the Pilot Program. All account information will be copied over from the user’s EPC administrative profile, including a list of associated consultants, discount rates, consortia members, and associated schools or libraries. If consortiums want to add new members to the group, they should do so through their E-Rate profile.
Users should review their school, library or consortia’s basic information that’s been pulled from the EPC administrative profile. If you need make changes to any of this basic information, contact the E-Rate Customer Service Center (CSC) at (888) 203-8100 between 8 a.m. and 8 p.m. ET Monday through Friday. Users will also designate a main contact person (themselves or someone else) and provide their contact information.
