Health Care Provider (HCP) Information

• For each HCP affiliated with the application, you may provide, but are not required to, the following information in the HCP Tab of Application:
  • Health care provider name
  • Facility name
  • If the facility is a hospital
  • Street address, city, state, and county
  • FCC Registration Number (FRN)
  • HCP Number
  • Eligibility Type
  • National Provider Identifier (NPI)
  • Total patient population
  • Estimated number of patients to be served by the funding request (and supporting documentation)
