Request Invoice Copy

Note: As of October 2024, USAC no longer sends paper invoices. However, you may still request copies of paper invoices sent before this change.

To request a copy of your invoice, contact the USAC Customer Operations team via email with the following information included in the body of the email:

  • Requestor’s name
  • Filer ID
  • Date of statement

If your name is not listed on the current FCC Form 499-A, FCC Form 499-Q, or current invoice, USAC cannot provide you with a copy of an invoice.

If you are listed on one of these forms, and you include the above information, you should receive a copy of your invoice via email within 24-48 hours of the time the request was received by USAC.
